Canvis supports two different types of survey forms. Layer based "Follow up questions" and page based embedded forms.

Combining these traditional forms with maps is what makes canvis.app projects so unique and insightful!

Layer based forms - "follow-up questions"

Follow-up questions appear when a user makes a contribution to a collaborative map. Every map layer can have it's own set of questions. If you aren't familiar with map layers, read this first: Map Layers

These forms are helpful for describing the data that is being added to the map. For example, if you are asking users to mark 🌲trees on a map, you might consider asking these follow up questions:

How to add follow-up questions to a layer

Head to your project and click "Edit" to open the builder tool bar.

From the builder toolbar, click "Layers" to view the layers list.

For each layer, you will see a yellow icon— this opens the survey form editor for that layer.

Page based forms

Unlike follow-up questions for map data, page-based forms are useful for asking general questions about the person, or anything that doesn't apply to map data.
